Terms and Conditions - Adelaide 36ers Survey Competition

        General

1. ‘The Promoter’ is The Hospital Research Foundation (ABN 86 248 830 078) of 62 Woodville Road, Woodville SA 5011. Telephone number (08) 8244 1100.
   

   Conditions of Entry

2. Winner must be an Adelaide 36ers 2022/23 Season Member and have received a copy of the survey link directly to their own email inbox.
3. The winner must live in South Australia and be over the age of 15 years old.
4. Closing date of entry and completion of the survey is Wednesday 26 April 2023 by 5.00pm AEST. After this date no further entries will be valid.
5. Only those members who have completed the full survey will go into the draw to win. A valid entry is considered only when all survey questions have been answered and the required contact details completed; First name, Surname, email address and mobile phone number.
6. Entry is permitted once only per person.
7. The Promoter accepts no responsibility for any late, lost or misdirected entries.
   

   
   Draw and Notification

8. The winner will be the first one (1) valid entry submitted in accordance with these terms and conditions that is drawn by The Hospital Research Foundation by random computer selection at 10:00am on Thursday 27 April 2023 at 62 Woodville Road, Woodville SA 5011. One (1) winner will be drawn from the pool of entrants, with a total of one (1) prize awarded.
9. One (1) major winner will win a signed 36ers jersey, THRF basketball and a $50 Zorich Group voucher.
10. The winner will be notified through a phone call and email within one week of the draw.
11. No cash alternative to the prizes will be offered.
12. Prize to be collected from The Hospital Research Foundation, 62 Woodville Road, Woodville. If shipping of the prize is requested by the winner, it will be arranged at the winner’s cost.
13. Prizes will be awarded to person named in the entry.
14. Should an entrant’s contact details change during the promotional period, it is the entrant’s responsibility to notify the Promoter. A request to access or modify any information provided in an entry should be directed to the Promoter.
    

    Use of Data Collected

15. The winner agrees to the use of his/her name and image in any publicity material.
16. You are providing your information to the Adelaide 36ers and The Hospital Research Foundation only.
17. Any personal data relating to the winner or any other entrants will be used solely for the purpose of this survey competition and in accordance with current Australian data protection legislation and will not be disclosed to a third party without the entrant’s prior consent.
18. The Promoter reserves the right to request winners to provide proof of identity, proof of residency at the nominated prize delivery address and/or proof of entry validity or proof of registration ownership in order to claim a prize. Proof of identification, residency and entry considered suitable for verification is at the discretion of the Promoter. In the event that a winner cannot provide suitable proof, the winner will forfeit the prize in whole and no substitute will be offered.
19. By entering this competition, an entrant is indicating his/her acceptance and agreement to be bound by these terms and conditions.
